Protection of Wages on Insolvency Ordinance (Cap. 380)

1. There is no prescribed form under this ordinance
2. For the following provisions exempted under Schedule 1 of the Electronic Transactions Ordinance, please note that only paper-based submission will be accepted

Exempted provision	Required submission
Section 15; and  Section 16(2)(a)	Employees whose employers have become insolvent and hence have not cleared outstanding wages, wages in lieu of notice and severance payments may apply for ex gratia payment from the Protection of Wages on Insolvency Fund.  An application should be made with the Labour Department in a specified form with a statutory declaration.
