e-Form 3 – Notice of Conducting Safety Audit by Registered Safety Auditor

The Labour Department (LD) has launched the “Notice of Conducting Safety Audit by Registered Safety Auditor” (i.e. Form 3 under the Factories and Industrial Undertakings (Safety Management) Regulation) online version (hereafter called e-Form 3) for Registered Safety Auditor (RSA) to submit notification of the proposed safety audit through online system to LD.

Section 18 of the Factories and Industrial Undertakings (Safety Management) Regulation stipulates that a RSA shall give notice in the approved form not less than 14 days before to LD of the date on, and the time and place at which a safety audit will commence.  The e-Form 3 provides a more convenient way for RSA to submit notification of safety audit to LD.  RSA could choose the traditional paper Form 3 or the e-Form 3 for submission to best suit the operational needs.

The e-Form 3 is available in English, Traditional Chinese and Simplified Chinese.  RSA can use the Government e-Forms under GovHK of the “iAM Smart” mobile application, “OSH 2.0” mobile application or directly access via https://eform.cefs.gov.hk/form/ld0003/en/ to complete and submit the notice and relevant documents.
