Download the blank E-Form (*.afs) and launch E-Form Filler/Viewer
(please see Section 3.2 and Section 6.1 of User Manual)

Read the "Important Notice" and "Instruction" of the E-Form and then input the data into the E-Form
(please see Section 6.2.5 and Section 6.2.8 of User Manual)

Incorporate attachments (if any) into the E-Form
(please see Section 6.2.9 of User Manual)

Apply digital signature(s) to theE-Form (if necessary) by a valid digital certificate
(please see Section 6.2.10 of User Manual and the "Instruction" section of individual E-Forms)

Indicate the area information
(only applicable for certain E-Forms)
(please see Section 6.2.11 of User Manual and the "Instruction" section of individual E-Forms)

Encrypt the E-Form (optional)
(please see Section 6.2.13 of User Manual)

Submit the completed E-Form by e-mail
(please see Section 6.2.14 of User Manual).
For ease of processing of your E-Form, please include only one E-Form in each e-mail submission.
